Urine albumin-creatinine ratio (UACR) is clinically indicated for individuals with chronic kidney disease, diabetes, or hypertension.

Hemoglobin A1c (HbA1c) is clinically indicated for individuals with diabetes, prediabetes, overweight, or obesity, or those with history of gestational diabetes.

ZIP code is used to retrieve the SDI (Social Deprivation Index) decile. Lower SDI decile values are more favorable. A little more detail on this is at this page.
